Resolution:
In the 1970's and early 1980's, Square D Company manufactured and sold Ground Fault Receptacles. These were obsoleted in the 1980's. The unique feature of the GFSR receptacles was that their shallow depth did not require deep wall boxes (see the below image). If an existing GFSR receptacle is being replaced, always use a receptacle that offers GFCI protection (not a standard receptacle). Another option is to add ground fault protection to the entire circuit by using a circuit breaker with GFCI protection. Contact your local electrician for assistance.
